Insurance

We are happy to bill your insurance company for you. However, at the time of service, we require the difference between the benefit and the charges.
Please bring a copy of your insurance benefits, as well as your benefits billing form, if required.
We are a participating provider for Medicare and Medicaid (DSHS).
We accept cash, check, Visa, MasterCard and The Discover Card.

Appointments

Our hours are Monday through Friday from 8:00 - 5:00. Evening appointments may be scheduled.

Please bring your current glasses or contacts, a list of medications you are taking, and your insurance benefits.

Please notify us at least 24 hours in advance if you're unable to keep your appointment.

Dr. Michael DeRosier was born and raised in Klamath Falls, Oregon. He received his Bachelor of Science degrees from Oregon State University and from Pacific University. He earned his Doctorate of Optometry in 1983 at the Pacific University College of Optometry. He has practiced in Deer Park since 1983.

Dr. DeRosier and his wife, Kathy, have two daughters. All enjoy the outdoor activities offered in the Inland Empire.
